This will help us to be more efficient by making an appointment available to you when you need it the most.

Pharmacy and Self Care

Think Pharmacy First

Many conditions can be treated without the need to see your GP.

The pharmacist will offer you advice, treatment or refer you to a GP or another healthcare professional if needed.

Pharmacies will update your GP health record.

If you are not within these age ranges, a pharmacist can still offer advice, but you may need to see a GP for treatment.

Please note on weekends online self-referral may take longer as the practice is closed and will not look at your assessment until we are open, for quicker response please walk into your local pharmacy.

Ways to book

1. Download the NHS App:
The new, simple and secure way to access a range of NHS services on your smartphone or tablet.

2. Request an appointment online:
You will be asked questions to assess your symptoms and offered online advice.

3. Telephone: 020 3818 8270:
Our phone lines are very busy but our receptionists are doing their best to answer your call as quickly as possible. You will need to tell the receptionist what your call is for so our doctors can prioritise your care. If you don’t want to tell the receptionist your symptoms, use option 2

Sickness Certificates (Fit Notes)

You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Please complete our secure online form. Someone at the practice will book your appointment and notify you within 48 hours (not including weekends or bank holidays).

Booking a Blood Test

Home Visits

We encourage our patients to come to the surgery, where we have the proper equipment and facilities available. But we do appreciate this is not always possible. If you do need a home visit, you can help us by calling reception before 10am.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls.

You can also be visited at home by a community nurse if you are housebound. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years. 

Change or Cancel an Appointment

Please give us as much notice as possible so we can offer your appointment to someone else.

  • 12pm to 5pm: Pre-booked Telephone Consultation appointments only (PCN cover provided by Brocklebank Practice, The Haider Pratice and St. Paul's Cottage Surgery). Reception is closed after 12.30pm

Life Threatening

Call 999 or go to A&E now if:

  • you or someone you know needs immediate help
  • you have seriously harmed yourself – for example, by taking a drug overdose

A mental health emergency should be taken as seriously as a medical emergency.

  • You have an urgent medical issue requiring on the day attention
