Changing your personal details

Change of address

  • If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
  • We are now able to keep already registered patients who move outside of our catchment area, as long as you still live within the borough of Wandsworth­.
  • Check you are in our catchment area

Change of name

  • You will need to upload proof that you have changed your name. This could be a scan or photo of your marriage or civil partnership certificate, deed poll or birth certificate.

If you live outside of Wandsworth, unfortunately we cannot accept your registration or keep you registered as a patient.

Out of Area Registration

For patients who live outside the practice catchment area but within the Borough of Wandsworth.

You will have access to most routine GP primary care services whlist maintaining continuity of care with the practice.

What is excluded

  • Home visits
  • Families with children under 16.
  • Patients requiring care from geographically based specialist services, including:
    • Complex community care packages.
    • Serious mental health services (e.g. Community Mental Health Teams).
    • Child safeguarding or Children's Social Care services.
  • Some referrals are restricted by a patient's home address.

Eligibility

  • Patients living within the Borough of Wandsworth
  • Subject to a clinical review by the practice
  • The practice may refuse or end out of area registration if it is no longer appropriate or safe.
  • Failure to attend required review appointments after repeated invitations may result in removal from the practice list.
  • If circumstances change and out of area registration becomes unsuitable, patients may be required to register with a GP closer to home.

If you do not agree to these terms, you simply need to register with a GP closer to your home address to get the full benefits of a GP service which they can offer to you within their catchment area.
